Overview
The Kent Community Continuum of Care Program
is a comprehensive treatment program for individuals
with severe psychiatric disabilities. The program
offers a broad range of community treatment
and outreach services to facilitate successful
daily living and sobriety. Emergency and crisis
intervention services are available 24 hours
a day.
Objective
The Kent Community Continuum of Care Program’s
mission is to help psychiatrically disabled
adults to achieve the self-sufficiency and independence
necessary to function successfully in the community.
Conversely, the goal is to minimize the time
spent by these individuals in institutions.
Population Served
Adults with severe psychiatric disabilities
who are in need of intensive treatment services
are eligible. Clients must be referred through
the State of Delaware Division of Substance
Abuse and Mental Health (DSAMH). Individuals
are referred through the Eligibility and Enrollment
Unit. These individuals live in Kent County,
Delaware. Referrals may be accepted also if
the individual resides in New Castle County,
Delaware if the residence is south of Rt. 199.
Individuals who live in the city of Milford,
Delaware can be referred even if they are on
the Sussex County side of the Mispillion River.
Fees & Charges
The Kent Community Continuum of Care Program
is a Medicaid provider. The program also accepts
third party insurance, although this coverage
is typically limited to physician services.
Kent CCCP is operated under contract with the
State of Delaware’s Division of Alcohol,
Drug Abuse and Mental Health (DSAMH).
